Field Trip Reservation Request

1. All events at Apple Holler are subject to availability.  An event may be rescheduled or cancelled by Apple Holler in which case buyer will be notified immediately and given a reasonable opportunity to reschedule or cancel.

2. Teachers and chaperones are responsible for the safety of the children. The activities of the children must be monitored by the chaperones while at Apple Holler.

3. As in all farming enterprises, weather plays a major role in determining the size of a crop. At times, guests may need to chose an apple from a bin rather than trees.

Communication Policy: By signing and completing my field trip request form, I allow Apple Holler to email me with information about this or upcoming events.  I understand that I can opt out of this at any time.

Wisconsin Agritourism Liability Act Warning: A person who observes or participates in an agricultural tourism activity on this property assumes the risks inherent in the agricultural tourism activity.  Risks inherent in the agricultural tourism activity may include conditions on the land, the unpredictable behavior of farm animals, the ordinary dangers associated with equipment used in farming operations, and the potential that a participant in the agricultural tourism activity may act in a negligent way that may contribute to injury or death.  The agricultural tourism provider is not liable for the injury or death of a person involved in a agricultural tourism activity resulting from those inherent risks.  AG-Tourism Limited Liability Law-Wisconsin Act 269.

Warning: Play at your own risk.  Adult supervision is required for children under 16 years old.  Attraction attendants may not be on duty at all times.

Covid-19,Communicable & Zoonotic Disease Liability Waiver: By visiting Apple Holler, you fully acknowledge that there is an inherent risk of exposure to COVID-19, along with other common communicable and zoonotic diseases. Further, you fully accept such risk and agree to hold harmless Apple Holler, along with its owners, staff, and any affiliates should any issue, injury, illness or death arise for any reason to yourself, any member of your family, or any member in attendance with you.

By acknowledging below, I agree for all of those in my party and agree to hold harmless and do forever release Apple Holler for all current and future liability related to this visit.
